Medical Checks for Studying in China: The Physical Examination Record
An admission letter is only the first step. Students planning to study in China for more than 180 days must also complete a medical examination before arrival, submit the Foreigner Physical Examination Record, and have it verified again after entering China. A form that is filled in incorrectly or missing a seal can hold up both the visa and the residence permit.
- Who needs it: anyone planning to study in China for more than 180 days and applying for an X1 visa must complete a medical examination before arrival and submit the Foreigner Physical Examination Record. For short-term study of no more than 180 days, a medical certificate as required by the visa authority is sufficient.
- Where to get the form: from a Chinese embassy or consulate, or by downloading it from official channels such as the Chinese Service Center for Scholarly Exchange. The form must be completed in Chinese or English.
- Signature and seal requirements: the form must carry the applicant's photo with the hospital's official seal over it, a conclusion, the doctor's signature and the hospital seal, and a clearly legible issue date. Forms with missing items will be sent back for additional tests.
- Original reports must be attached: the examination must follow the items listed on the form, with original reports or laboratory slips attached, including the chest X-ray report or film, ECG tracing or report, laboratory results covering at least HIV and syphilis testing, and ultrasound reports.
- Institution and validity: the examination and the issuing of the report must be carried out by a local public hospital. If done at a private hospital or clinic, the report should first be notarised locally. The form is valid for six months from the date of issue.
- Verification after arrival: those coming to study in China for the first time and staying for one year or more must take their passport (with the X1 visa page), admission letter and JW201 or JW202 form to an international travel healthcare centre to have the record verified and to obtain the International Travel Health Certificate.
Timing: the medical report is valid for six months, so an examination done too early expires, and one done too late misses the visa window. A workable sequence is to book the examination as soon as the admission letter and the JW201 or JW202 form arrive, allowing two to four weeks for the report to be issued, for notarisation if needed, and for visa processing.
The five most common reasons a form is rejected: the photo does not carry the hospital's seal; only the form is submitted with no X-ray film or original laboratory slips; the blood tests omit HIV or syphilis; a report from a private clinic has not been notarised; and the form is completed in a language other than Chinese or English. Any one of these can require a fresh examination, which often means missing the start of term.
How the examination relates to the residence permit: verification after arrival normally happens before the residence permit application, and most university international student offices announce the time, place and documents for verification centrally. Students entering on an X1 visa must apply for a residence permit within 30 days of entry, so the two processes need to be lined up; do not wait for verification to finish before preparing the residence permit paperwork.
If the original form is ruled invalid, the only remedy is to be examined again. It is worth spending an extra half day checking every seal and signature position before departure rather than hoping to sort it out after arrival.
